- Évaluation du débit de filtration glomérulaire par l’une des 3 formules suivantes : Cockcroft, MDRD ou CKD-Epi
- Hémodialyse : compte tenu de son élimination, le produit doit être administré après la séance les jours d’hémodialyse
- DPCA : Dialyse péritonéale continue ambulatoire
